Registration & Order Forms
When visiting the Misermart.com web site and other Misermart affiliated sites, in order to join, make a purchase, register to use certain services, enter a contest, respond to a survey or
update your information you may be asked to provide certain personally identifiable information about yourself, such as your name, e-mail address, physical address, telephone number, date of birth, mother's maiden name or social
